Initial implementation

Provides an initial implementation of the library.

Publication to test PyPI via CI at https://gitlab.developers.cam.ac.uk/uis/devops/lib/fastapi-proxiedheadersmiddleware/-/jobs/354809 and it can be seen on PyPI at https://test.pypi.org/project/fastapi-proxiedheadersmiddleware/0.9.0/.

I've added the limited-scope TEST_PYPI_API_TOKEN CI variable but waiting until merge to deploy fully to live PyPI.
