This MR only includes documentation changes.

Changes:

Reference the v9 migration guide in the README and Changelog.
Added the change in default monitoring behaviour to the list of breaking changes in the CHANGELOG. disable_monitoring was replaced by enable_monitoring but the default value was kept as false.
