Add contact preference questions

Dr Catherine Pitt requested to merge 80-question-about-department-extension-number-should-be-optional into main

Adds two questions to the person/role details form about contact preferences and rewords the department telephone number(s) question, which is now allowed to be blank if another contact preference is set.
