In order to empower us as individual teams members background of the issue is useful, and also why certain decisions are made and why. The why will helps us advise and seek solutions rather than just do!

Background
Where does the request stem from, User needs, Stakeholder need, decision by committee etc
Decision reasoning
Why is this important, Why was decision made
Change of decision
Why was decision changed, who does it impact. If impact to work and capacity, was this communicated to stakeholders/decision makers
