Update DAR app for IT role tracking in the database

This needs to be updated to insert the appropriate IT role in the database for people whose accounts are being extended. Only first and second leaver extensions and ssh-only access should be grantable here. This therefore cannot be done until we have implemented the ssh-only access role's requirements.
